€‚For the podcast associated with this article, please visit https://academic.oup.com/eurheartj/pages/Podcasts.This Focus Issue on heart failure (HF) provides novel clinically relevant information on sodium–glucose co-transporter-2 (SGLT2) inhibitors which, initially can u buy cipro over the counter proposed for the treatment of type 2 diabetes mellitus (T2D), have been found to improve the outcome of HF with reduced ejection fraction (HFrEF) when administered on the top of drugs known to improve the outcome of HF and are recommended in current European Guidelines.1,2Acording to modelling estimates, when compared with no neurohormonal blockade, the use of a broad-based combination of disease-modifying drugs at target doses in patients with HF may reduce the risk of death by as much as 75%. It is surprising that in spite of this powerful therapeutic armamentarium, <1% of patients with chronic HF are currently receiving recommended drugs at doses that have been shown to prolong life.3 The issue opens with a Current Opinion article entitled ‘Totality of evidence in trials of sodium–glucose co-transporter-2 inhibitors in the patients with heart failure with reduced ejection fraction. Implications for clinical can u buy cipro over the counter practice’ by Milton Packer from the Baylor University Medical Center at Dallas in Texas, USA and colleagues. The authors provide a perspective on the totality of evidence with SGLT2 inhibitors in patients with HFrEF.4 This paper is the first to issue a call for a major change in clinical practice based on the concordant results of DAPA-HF and EMPEROR-Reduced trials.

The analyses and interpretations that are presented in this manuscript will undoubtedly generate considerable discussion and debate for a long time.Concern about hypotension often leads to withholding of beneficial therapy in patients with HFrEF. In a clinical research manuscript entitled ‘Effect of dapagliflozin according to baseline systolic blood pressure in the Dapagliflozin and Prevention of Adverse Outcomes in Heart Failure trial (DAPA-HF)’ John McMurray from the can u buy cipro over the counter Western Infirmary in Glasgow, UK and colleagues on behalf of the DAPA-HF Investigators and Committees evaluated the efficacy and safety of dapagliflozin according to baseline systolic blood pressure (SBP) in DAPA-HF trial.5 Key inclusion criteria were. New York Heart Association (NYHA) class II–IV, left ventricular ejection fraction (LVEF) ≤40%, elevated N-terminal probrain natriuretic peptide (NT-proBNP) level, and SBP ≥95 mmHg. The primary outcome was a composite of worsening HF or cardiovascular death.

The efficacy can u buy cipro over the counter and safety of dapagliflozin was examined using SBP as both a categorical and a continuous variable. The placebo-corrected reduction in SBP from baseline to 2 weeks with dapagliflozin was –2.54 mmHg. The benefit and safety of dapagliflozin were consistent across the range of SBP. Study drug discontinuation did not differ between dapagliflozin and placebo across can u buy cipro over the counter the SBP categories examined. The authors conclude that dapagliflozin had a small effect on SBP in patients with HFrEF and was superior to placebo in improving outcomes, and well tolerated, across the range of SBP included in DAPA-HF.

The manuscript is accompanied by an Editorial by Francesco Cosentino from the University Hospital Solna in Stockholm, Sweden who comments that altogether, the results of the current post-hoc analysis demonstrating efficacy and safety of dapagliflozin regardless of SBP values might significantly contribute to foster the implementation of dapagliflozin use in HF clinical practice by dissipating any potential safety concern linked with its hypotensive effects.6In a clinical research article entitled ‘A randomized controlled trial of dapagliflozin on left ventricular hypertrophy in people with type two diabetes. The DAPA-LVH trial’, Chim Lang from the University of Dundee in the UK and colleagues tested the hypothesis that dapagliflozin may regress left ventricular hypertrophy can u buy cipro over the counter (LVH) in people with T2D.7 The authors randomly assigned 66 patients with T2D, LVH, and controlled blood pressure to receive dapagliflozin 10 mg once daily or placebo for 12 months. The primary endpoint was change in absolute left ventricular mass (LVM), assessed by cardiac magnetic resonance imaging (MRI). In the intention-to-treat analysis, dapagliflozin significantly reduced LVM compared with placebo, with an absolute mean change of –2.82 g.

Additional sensitivity analysis adjusting for baseline LVM, can u buy cipro over the counter baseline blood pressure, weight, and SBP change showed the LVM change to remain statistically significant. Dapagliflozin significantly reduced pre-specified secondary endpoints including ambulatory 24-h SBP, nocturnal SBP, body weight, visceral adipose tissue, subcutaneous adipose tissue, insulin resistance, and high-sensitivity C-reactive protein. See pages 3421–3432).Lang and colleagues conclude that dapagliflozin treatment significantly reduced LVM in patients with T2D and can u buy cipro over the counter LVH. The regression of LVM suggests that dapagliflozin can initiate reverse remodelling and changes in left ventricular structure that may partly contribute to cardioprotective effects of dapagliflozin. This manuscript is accompanied by an Editorial by Francesco Paneni from the University of Zurich in Switzerland and colleagues.8 They note that the above-mentioned effects of SGLT2 inhibitors set the ground for a possible beneficial effect of these drugs in patients with HFpEF, where microvascular dysfunction, cardiomyocyte inflammation, and cardiometabolic alterations take centre stage. While several landmark studies have long established that implantable cardioverter-defibrillator (ICD) therapy improves survival for primary prevention of sudden cardiac death ,9 risk stratification parameters and methods for this purpose are clinically underused. In a clinical can u buy cipro over the counter research article entitled ‘Clinical effectiveness of primary prevention implantable cardioverter-defibrillators.

Results of the EU-CERT-ICD controlled multicentre cohort study’ Markus Zabel from the Universitätsmedizin Göttingen in Germany and colleagues from the EU-CERT-ICD Study Investigators assessed the current clinical effectiveness of primary prevention by ICD therapy in a prospective investigator-initiated, controlled cohort study, conducted in 44 centres and 15 European countries. The study sought to assess current clinical effectiveness of primary prophylactic ICD implantation.10 The authors recruited 2327 patients with ischaemic or dilated cardiomyopathy and guideline indications for prophylactic ICD implantation. The primary endpoint was all-cause can u buy cipro over the counter mortality. Baseline and follow-up data from 2247 patients were analysable.

1516 patients with first ICD implantation (ICD group) and 731 patients without ICD serving as controls. Multivariable models and propensity scoring for adjustment were used can u buy cipro over the counter to compare the two groups for mortality. Adjusted mortality associated with ICD vs. Control was significantly lower (hazard ratio can u buy cipro over the counter 0.731).

Subgroup analyses indicated no ICD benefit in diabetics or in those aged ≥75 years. Figure 2Secondary efficacy endpoints comparing cardiosphere-derived cells and placebo at 6 months. See pages 3451--3458).The authors conclude that in contemporary ischaemic/dilated cardiomyopathy patients (LVEF ≤35%, narrow QRS), primary prophylactic ICD treatment was associated with a substantial reduction in mortality, although this improvement was not consistent across the whole population.

The manuscript is accompanied by an can u buy cipro over the counter Editorial by N.A. Mark Estes III from the Heart and Vascular Institute UPMC in Pittsburgh, Pennsylvania, USA.11 The authors note that clinicians should be mindful of available risk stratification models and subgroup analyses from the EU-CERT-ICD and other studies. It follows that the process of shared decision-making should include careful consideration of the patient’s wishes and values, with an individualized assessment of potential benefit and risks of primary prevention of sudden death by ICD implantation.Cardiosphere-derived cells (CDCs) are cardiac progenitor cells which exhibit disease-modifying bioactivity in various models of cardiomyopathy and in previous clinical studies of acute myocardial infarction (MI), dilated cardiomyopathy, and Duchenne muscular dystrophy.12,13 In a clinical research article entitled ‘Intracoronary ALLogeneic heart STem cells to Achieve myocardial Regeneration (ALLSTAR). A randomized, placebo-controlled, double-blinded trial’, Raj Makkar from the Cedars-Sinai Heart Institute in Los Angeles, California, USA and colleagues assessed the safety and efficacy of intracoronary administration of allogeneic CDCs in the multicentre, randomized, double-blind, placebo-controlled, intracoronary ALLogeneic Heart STem Cells to Achieve Myocardial Regeneration (ALLSTAR) trial.14 The authors enrolled patients 4 weeks to 12 months after MI, can u buy cipro over the counter with LVEF ≤45% and left ventricular LV scar size ≥15% of LVM by MRI.

A pre-specified interim analysis was performed when 6-month MRI data were available. The trial was subsequently stopped due to the low probability of detecting a significant treatment effect of CDCs based on the primary endpoint. Patients were can u buy cipro over the counter randomly allocated in a 2:1 ratio to receive CDCs or placebo in the infarct-related artery by the stop–flow technique. The primary safety endpoint was the occurrence, during 1-month post-intracoronary infusion, of acute myocarditis attributable to allogeneic CDCs, ventricular tachycardia- or ventricular fibrillation-related death, sudden unexpected death, or a major adverse cardiac event (death or hospitalization for HF or non-fatal MI).

The primary can u buy cipro over the counter efficacy endpoint was the relative percentage change in infarct size at 12 months post-infusion as assessed by contrast-enhanced cardiac MRI. Makkar and colleagues randomly allocated 90 patients to the CDC group and 44 to the placebo group. The mean baseline LVEF was 40% and the mean scar size was 22% of the LVM. No primary safety endpoint can u buy cipro over the counter events occurred.

There was no difference in the percentage change from baseline in scar size between CDC and placebo groups at 6 months. Compared with placebo, there were significant reductions in LV end-diastolic volume, LV end-systolic volume, and NT-proBNP at 6 months in CDC-treated patients. The authors conclude that intracoronary infusion of allogeneic CDCs in patients with post-MI left ventricular dysfunction was safe but did not reduce scar size relative to placebo at 6 months. The manuscript is accompanied by an Editorial by Francisco can u buy cipro over the counter Fernandez-Aviles from the Hospital General Universitario Gregorio Marañón in Madrid, Spain and colleagues.15 The authors feel that various points need to be better addressed before proceeding again to clinical trials, if we want to move the field of cardiovascular regenerative and reparative medicine forward, for the sake of the cardiovascular health of millions of patients.Treatment of pathological cardiac remodelling and subsequent HF represents an unmet clinical need. Long non-coding RNAs (lncRNAs) are emerging as crucial molecular orchestrators of disease processes including that of heart diseases.16,17 In a Basic Science article entitled ‘Targeting muscle-enriched long non-coding RNA H19 reverses pathological cardiac hypertrophy’, Thomas Thum from the Hannover Medical School in Germany, and colleagues report on the powerful therapeutic potential of the conserved lncRNA H19 in the treatment of pathological cardiac hypertrophy.18 Pressure overload-induced left ventricular cardiac remodelling revealed an up-regulation of H19 in the early phase, but a strong sustained repression upon reaching the decompensated phase of HF.

The translational potential of H19 was highlighted by its repression in a large animal (pig) model of LVH, in diseased human heart samples, in human stem cell-derived cardiomyocytes, and in human engineered heart tissue in response to afterload enhancement. Pressure overload-induced can u buy cipro over the counter cardiac hypertrophy in H19 knockout mice was aggravated compared with wild-type mice. In contrast, vector-based, cardiomyocyte-directed gene therapy using murine but also human H19 strongly attenuated HF even when cardiac hypertrophy was already established. Mechanistically, using can u buy cipro over the counter microarray, gene set enrichment analyses, and chromatin immunoprecipitation-DNA sequencing, the authors identified a link between H19 and prohypertrophic nuclear factor of activated T cells (NFAT) signalling.

H19 physically interacts with the polycomb repressive complex 2 to suppress H3K27 tri-methylation of the antihypertrophic Tescalcin locus which in turn leads to reduced NFAT expression and activity.Thum and colleagues conclude that H19 is highly conserved and down-regulated in failing hearts from mice, pigs, and humans. H19 gene therapy prevents and reverses experimental pressure overload-induced HF. H19 acts as an antihypertrophic lncRNA and represents a promising therapeutic target to combat pathological can u buy cipro over the counter cardiac remodelling. The manuscript is accompanied by an Editorial by Gianluigi Condorelli from the Humanitas University in Rozzano, Italy and colleagues.

The authors note that dysregulation of epigenetic mechanisms leading to aberrant loss of cardiomyocyte homeostasis is a critical point to consider in understanding the onset of cardiovascular pathologies. Thus exploiting lncRNAs as therapeutic agents in myocardial disease could pave the way for efficaciously combatting one of the greatest healthcare burdens worldwide.19With the advent of omics, an innovative inductive method has provided researchers with possible ways can u buy cipro over the counter new to monitor health and disease. This approach incorporates data from studies of the genome, transcriptome, proteome, and metabolome to focus on the assessment of a varied range of biomolecules.20 In a clinical review article entitled ‘Omics phenotyping in heart failure. The next frontier’ Antoni Bayes-Genis from the Cardiology Service, Hospital Universitari Germans Trias i Pujol in Badalona, Spain and colleagues provide a state-of-the-art review aiming to provide an up-to-date look at breakthrough omic technologies that are helping to unravel HF disease mechanisms and heterogeneity.21 Genomics, transcriptomics, proteomics, and metabolomics in HF are reviewed in depth.

In addition, there is a thorough, expert discussion regarding the value of omics in identifying novel disease pathways, advancing understanding of disease mechanisms, differentiating HF phenotypes, yielding biomarkers for diagnosis or prognosis, can u buy cipro over the counter or identifying new therapeutic targets in HF. The combination of multiple omics technologies may create a more comprehensive picture of the factors and pathophysiology involved in HF than achieved by either one alone, and provides a rich resource for predictive phenotype modelling. However, the successful translation can u buy cipro over the counter of omics tools as solutions to clinical HF requires that the observations are robust and reproducible, and can be validated across multiple independent populations to ensure confidence in clinical decision-making.This issue is also complemented by a Discussion Forum contribution. Health 2.0 Middle East Digital Event 2020, can you drink alcohol while taking cipro and flagyl running 29 November to 2 December 2020.Like the rest of the world during the cipro, healthcare organisations in Saudi Arabia are no longer centralised, but are now spread across remote work settings, making them vulnerable to varied threats from unsuitable hardware and non-firewalled systems. This has been in tandem with Saudi Arabia's national strategy for digital transformation that has incorporated five-year goals for which three executive plans were drawn from 2006 until 2022, with digital health as one of its core components. Although strategies have been put in place to launch and fortify technologies such as user data collection, augmented reality and telehealth, questions around adequate protection for evolving cybersecurity threats posed to these can you drink alcohol while taking cipro and flagyl technologies has certainly become a needed conversation in the current climate.

Dr Saif Abed, founding partner and director of cybersecurity advisory services at AbedGraham explains how he thinks Saudia Arabia's strong investment in digitalisation has raised awareness around cyber-attacks. "Any time a significant investment is made to accelerate digital transformation can you drink alcohol while taking cipro and flagyl you have to consider both benefits and risks associated with it. The strong track record of investment across the Middle East has clearly been successful when it comes to enhancing digital maturity and this now places the region in a strong position to make investments across people, processes and technology that can preserve patient safety and clinical services."At a time when the rest of the world is suffering from terrible cyber-attacks on healthcare and life sciences facilities, the Middle East has a strong foundation to counter this moving forward through increased awareness and action."Dr Abed will be moderating the 'Cybersecurity and Incident Response Masterclass' at the HIMSS &. Health 2.0 Middle East Conference on 1 December.Global cybersecurity can you drink alcohol while taking cipro and flagyl lessonsLooking globally at other regions and countries, there has been a rise in cyber-attacks since the cipro began, particularly in healthcare, with ransomware attacks targeting hospitals and research facilities.

According to a statement issued by global technology company Acronis, cybercriminals “will target the government agencies, healthcare facilities and medical professionals treating patients during the buy antibiotics can you drink alcohol while taking cipro and flagyl crisis,” after it found a surge in ransomware detections in Europe by up to seven per cent in the last week of February 2020, followed by a 10% increase the week after.Global attitudes and action towards cybersecurity prevention has highlighted areas we can learn from in regards to successful approaches to cyber-attacks. "Globally, we must always be learning from one other irrespective of the region since we all have the same goal of preventing patient harm."Particularly interesting areas that I think the Middle East is in a position to learn more about, based on developments in the US and UK, include how to address medical IoT security and clinical risk analysis respectively," explains Abed."There are many emerging regulations, advisory statements and technologies in the market that are addressing medical IoT which the Middle East can act on. In terms of clinical risk analysis of network threats and vulnerabilities, this is an can you drink alcohol while taking cipro and flagyl exciting emerging area that can turbocharge risk management for CISOs, CIOs and CMIOs across their organisations," adds Abed.Digital maturity outgrowing security maturity The rapid growth of digital maturity outgrowing security maturity during the cipro has also been an area of concern for many healthcare organisations. "This is something I coined in a graphic I call the’ Healthcare Maturity Paradox’," said Abed."Essentially, when digital maturity increases it’s normally not matched by an equal investment in security maturity and the gaps between these two areas, or lines on a chart, is what makes healthcare organisations attractive targets for attackers.

I call that the ‘Attacker’s Arbitrage Opportunity’."The best way to address this is can you drink alcohol while taking cipro and flagyl to always ensure that as part of an investment in a digital project that there is a baseline risk assessment conducted that captures clinical, organisational, financial and reputational damage risks and that investments are made based on this in people, processes and technology to preserve the security posture of a digital transformation initiative."Resources and raising awarenessMultiple resources are now available not just for technical members of the team but other staff members across the healthcare organisations to prevent ransomware attacks and to raise awareness around the topic. "We’re fortunate today to have more resources than ever before when it comes to cybersecurity," notes Abed. "HIMSS of course provides a treasure trove can you drink alcohol while taking cipro and flagyl of resources in this area. There are also a range of agencies that such as the EU Cybersecurity Agency, MITRE and the FDA that provide great information that is increasingly accessible at a non-technical level.

Other than that the best resource is often can you drink alcohol while taking cipro and flagyl your peer group so regional meetings and conferences are a great way to raise awareness and share best practice."Furthermore, as awareness and education on the topic grows, cybersecurity in healthcare will also need to consequently evolve. "Firstly, healthcare organisations over time are going to have greater transparency of what’s on their network, because you can’t control what you don’t know about."Secondly, cybersecurity will stop being a technical subject and instead will be seen through the lens of patient safety and measurable business outcomes. That means more proactive and constructive engagement with non-technical senior stakeholders," concludes Abed. You can find out more about the HIMSS &. Health 2.0 Middle East Digital Event 2020, running 29 November to 2 December 2020.Like the rest of the world during the cipro, healthcare organisations in Saudi Arabia are no longer centralised, but are now spread across remote can u buy cipro over the counter work settings, making them vulnerable to varied threats from unsuitable hardware and non-firewalled systems. This has been in tandem with Saudi Arabia's national strategy for digital transformation that has incorporated five-year goals for which three executive plans were drawn from 2006 until 2022, with digital health as one of its core components. Although strategies have can u buy cipro over the counter been put in place to launch and fortify technologies such as user data collection, augmented reality and telehealth, questions around adequate protection for evolving cybersecurity threats posed to these technologies has certainly become a needed conversation in the current climate.

Dr Saif Abed, founding partner and director of cybersecurity advisory services at AbedGraham explains how he thinks Saudia Arabia's strong investment in digitalisation has raised awareness around cyber-attacks. "Any time a significant investment is made to accelerate digital transformation can u buy cipro over the counter you have to consider both benefits and risks associated with it. Health 2.0 Middle East Conference on 1 December.Global cybersecurity lessonsLooking globally at other regions and countries, there has been a rise in cyber-attacks since can u buy cipro over the counter the cipro began, particularly in healthcare, with ransomware attacks targeting hospitals and research facilities. According to a statement issued by global technology company Acronis, cybercriminals “will target the government agencies, healthcare facilities and medical professionals treating patients during the buy antibiotics crisis,” after can u buy cipro over the counter it found a surge in ransomware detections in Europe by up to seven per cent in the last week of February 2020, followed by a 10% increase the week after.Global attitudes and action towards cybersecurity prevention has highlighted areas we can learn from in regards to successful approaches to cyber-attacks.

In terms of clinical risk analysis of network threats and vulnerabilities, this is an exciting emerging area that can turbocharge risk management for CISOs, CIOs and can u buy cipro over the counter CMIOs across their organisations," adds Abed.Digital maturity outgrowing security maturity The rapid growth of digital maturity outgrowing security maturity during the cipro has also been an area of concern for many healthcare organisations. I call that the ‘Attacker’s Arbitrage Opportunity’."The best way to address this is to always ensure that as part of an investment in a digital project that there is a baseline risk assessment conducted that captures clinical, organisational, financial and reputational damage risks and that investments are made based on this in people, processes and technology to preserve the security posture of a digital transformation initiative."Resources and raising awarenessMultiple resources are now available not just for technical members of the team but can u buy cipro over the counter other staff members across the healthcare organisations to prevent ransomware attacks and to raise awareness around the topic. "We’re fortunate today to have more resources than ever before when it comes to cybersecurity," notes Abed.

"HIMSS of course provides a can u buy cipro over the counter treasure trove of resources in this area. There are also a range of agencies that such as the EU Cybersecurity Agency, MITRE and the FDA that provide great information that is increasingly accessible at a non-technical level. Other than that the best resource is often can u buy cipro over the counter your peer group so regional meetings and conferences are a great way to raise awareness and share best practice."Furthermore, as awareness and education on the topic grows, cybersecurity in healthcare will also need to consequently evolve. "Firstly, healthcare organisations over time are going to have greater transparency of what’s on their network, because you can’t control what you don’t know about."Secondly, cybersecurity will stop being a technical subject and instead will be seen through the lens of patient safety and measurable business outcomes. That means more proactive and constructive engagement with non-technical can u buy cipro over the counter senior stakeholders," concludes Abed. You can find out more about the HIMSS &.

IStock Share Fast Facts New @HopkinsMedicine study finds African-American women with common form of hair loss at increased risk of uterine fibroids - Click to Tweet New study in @JAMADerm shows most common form of alopecia (hair loss) in African-American women associated with higher risks of uterine fibroids - Click to Tweet In a study of medical records gathered on hundreds of thousands of African-American women, Johns Hopkins researchers say they have evidence that women with a common form of hair loss have an increased chance of developing uterine leiomyomas, or fibroids. In a report on the research, published in the December 27 issue of JAMA Dermatology, the researchers call on physicians who treat women with central centrifugal cicatricial alopecia (CCCA) to make patients aware that they may be at increased risk for fibroids and should be screened for the condition, particularly if they have symptoms such as heavy bleeding and pain. CCCA predominantly affects black women and is the where can you get cipro most common form of permanent alopecia in this population. The excess scar tissue that forms as a result of this type of hair loss may also explain the higher risk for uterine fibroids, which are characterized by fibrous growths in the lining of the womb.

Crystal Aguh, M.D., assistant professor of dermatology at the Johns Hopkins University School of Medicine, says the scarring associated with CCCA is similar to the scarring associated with excess fibrous tissue elsewhere in the body, a situation that may explain why women with this type of hair loss are at a higher risk for fibroids.People of African descent, she notes, are more prone to develop other disorders of abnormal scarring, termed fibroproliferative disorders, such as keloids (a type of where can you get cipro raised scar after trauma), scleroderma (an autoimmune disorder marked by thickening of the skin as well as internal organs), some types of lupus and clogged arteries. During a four-year period from 2013-2017, the researchers analyzed patient data from the Johns Hopkins electronic medical record system (Epic) of 487,104 black women ages 18 and over. The prevalence of those with fibroids was compared in patients with where can you get cipro and without CCCA. Overall, the researchers found that 13.9 percent of women with CCCA also had a history of uterine fibroids compared to only 3.3 percent of black women without the condition.

In absolute numbers, out of the 486,000 women who were reviewed, 16,212 had fibroids.Within that population, 447 had CCCA, of which 62 had fibroids. The findings translate where can you get cipro to a fivefold increased risk of uterine fibroids in women with CCCA, compared to age, sex and race matched controls. Aguh cautions that their study does not suggest any cause and effect relationship, or prove a common cause for both conditions. €œThe cause where can you get cipro of the link between the two conditions remains unclear,” she says.

However, the association was strong enough, she adds, to recommend that physicians and patients be made aware of it. Women with this type of scarring alopecia should be screened not where can you get cipro only for fibroids, but also for other disorders associated with excess fibrous tissue, Aguh says. An estimated 70 percent of white women and between 80 and 90 percent of African-American women will develop fibroids by age 50, according to the NIH, and while CCCA is likely underdiagnosed, some estimates report a prevalence of rates as high as 17 percent of black women having this condition. - Click to Tweet The “mutational burden,” or the number of mutations present in a tumor’s DNA, is a good predictor of whether that where can you get cipro cancer type will respond to a class of cancer immunotherapy drugs known as checkpoint inhibitors, a new study led by Johns Hopkins Kimmel Cancer Center researchers shows. The finding, published in the Dec. 21 New England Journal of Medicine, could be used to guide future clinical trials for these drugs. Checkpoint inhibitors are a relatively new class of drug that helps the immune where can you get cipro system recognize cancer by interfering with mechanisms cancer cells use to hide from immune cells.

As a result, the drugs cause the immune system to fight cancer in the same way that it would fight an . These medicines have had remarkable success in treating some types of cancers that where can you get cipro historically have had poor prognoses, such as advanced melanoma and lung cancer. However, these therapies have had little effect on other deadly cancer types, such as pancreatic cancer and glioblastoma. The mutational burden of certain tumor types has previously been proposed as an explanation for why certain cancers respond better than where can you get cipro others to immune checkpoint inhibitors says study leader Mark Yarchoan, M.D., chief medical oncology fellow.

Work by Dung Le, M.D., associate professor of oncology, and other researchers at the Johns Hopkins Kimmel Cancer Center and its Bloomberg~Kimmel Cancer Institute for Cancer Immunotherapy showed that colon cancers that carry a high number of mutations are more likely to respond to checkpoint inhibitors than those that have fewer mutations. However, exactly how big an effect the mutational burden has on outcomes to immune checkpoint inhibitors across many different cancer types was unclear. To investigate this question, Yarchoan and colleagues Alexander Hopkins, Ph.D., research fellow, and Elizabeth Jaffee, M.D., co-director of where can you get cipro the Skip Viragh Center for Pancreas Cancer Clinical Research and Patient Care and associate director of the Bloomberg~Kimmel Institute, combed the medical literature for the results of clinical trials using checkpoint inhibitors on various different types of cancer. They combined these findings with data on the mutational burden of thousands of tumor samples from patients with different tumor types.

Analyzing 27 different where can you get cipro cancer types for which both pieces of information were available, the researchers found a strong correlation. The higher a cancer type’s mutational burden tends to be, the more likely it is to respond to checkpoint inhibitors. More than half of the differences in where can you get cipro how well cancers responded to immune checkpoint inhibitors could be explained by the mutational burden of that cancer. €œThe idea that a tumor type with more mutations might be easier to treat than one with fewer sounds a little counterintuitive.

It’s one of those things that doesn’t sound right when you hear it,” says Hopkins. €œBut with immunotherapy, the more mutations you have, the more chances the immune system has to recognize the tumor.” Although this finding held true for the vast majority of cancer types they studied, there were some outliers in their where can you get cipro analysis, says Yarchoan. For example, Merkel cell cancer, a rare and highly aggressive skin cancer, tends to have a moderate number of mutations yet responds extremely well to checkpoint inhibitors. However, he explains, this cancer type is often caused by a cipro, which seems to encourage where can you get cipro a strong immune response despite the cancer’s lower mutational burden.

In contrast, the most common type of colorectal cancer has moderate mutational burden, yet responds poorly to checkpoint inhibitors for reasons that are still unclear. Yarchoan notes that these findings could help guide clinical trials where can you get cipro to test checkpoint inhibitors on cancer types for which these drugs haven’t yet been tried. Future studies might also focus on finding ways to prompt cancers with low mutational burdens to behave like those with higher mutational burdens so that they will respond better to these therapies. He and his colleagues plan to extend this line of research by investigating whether mutational burden might be a good predictor of whether cancers in individual patients might respond well to this class of immunotherapy drugs.

€œThe end goal is precision medicine—moving beyond what’s true for big groups of patients to see whether we can use this information to help any given patient,” where can you get cipro he says. - Click to Tweet The “mutational burden,” or the number of mutations present in a tumor’s DNA, is a good predictor of whether that cancer type will respond to a class of cancer immunotherapy drugs known as checkpoint inhibitors, a new study led by Johns Hopkins Kimmel Cancer Center researchers shows. The finding, published in the Dec. 21 New England Journal of Medicine, could be used to guide future clinical trials for these drugs. Checkpoint inhibitors are a relatively new class of drug that helps the immune system recognize cancer by interfering with mechanisms cancer cells use to hide from immune cells. As a result, the drugs cause the immune system to fight cancer in the same way that it would fight an .

These medicines have had remarkable success in treating some types of cancers that historically have had poor prognoses, such as advanced melanoma and lung cancer. However, these therapies have had little effect on other deadly cancer types, such as pancreatic cancer and glioblastoma. The mutational burden of certain tumor types has previously been proposed as an explanation for why certain cancers respond better than others to immune checkpoint inhibitors says study leader Mark Yarchoan, M.D., chief medical oncology fellow.

To investigate this question, Yarchoan and colleagues Alexander Hopkins, Ph.D., research fellow, and Elizabeth Jaffee, M.D., co-director of the Skip Viragh Center for Pancreas Cancer Clinical Research and Patient Care and associate director of the Bloomberg~Kimmel Institute, combed the medical literature for the results of clinical trials using checkpoint inhibitors on various different types of cancer. They combined these findings with data on the mutational burden of thousands of tumor samples from patients with different tumor types. Analyzing 27 different cancer types for which both pieces of information were available, the researchers found a strong correlation. The higher a cancer type’s mutational burden tends to be, the more likely it is to respond to checkpoint inhibitors. More than half of the differences in how well cancers responded to immune checkpoint inhibitors could be explained by the mutational burden of that cancer.

€œThe idea that a tumor type with more mutations might be easier to treat than one with fewer sounds a little counterintuitive. It’s one of those things that doesn’t sound right when you hear it,” says Hopkins. €œBut with immunotherapy, the more mutations you have, the more chances the immune system has to recognize the tumor.” Although this finding held true for the vast majority of cancer types they studied, there were some outliers in their analysis, says Yarchoan. For example, Merkel cell cancer, a rare and highly aggressive skin cancer, tends to have a moderate number of mutations yet responds extremely well to checkpoint inhibitors. However, he explains, this cancer type is often caused by a cipro, which seems to encourage a strong immune response despite the cancer’s lower mutational burden.

In contrast, the most common type of colorectal cancer has moderate mutational burden, yet responds poorly to checkpoint inhibitors for reasons that are still unclear. Yarchoan notes that these findings could help guide clinical trials to test checkpoint inhibitors on cancer types for which these drugs haven’t yet been tried. €œThe end goal is precision medicine—moving beyond what’s true for big groups of patients to see whether we can use this information to help any given patient,” he says.

CCCA predominantly affects black women and is the most common form of permanent alopecia in this population.

The excess scar tissue that forms as a result of this type of hair loss may also explain the higher risk for uterine fibroids, which are characterized by fibrous growths in the lining of the womb. Crystal Aguh, M.D., assistant professor of dermatology at the Johns Hopkins University School of Medicine, says the scarring associated with CCCA is similar to the scarring associated with excess fibrous tissue elsewhere in the body, a situation that may explain why women with this type of hair loss are at a higher risk for fibroids.People of African descent, she notes, are more prone to develop other disorders of abnormal scarring, termed fibroproliferative disorders, such as keloids (a type of raised scar after trauma), scleroderma (an autoimmune disorder marked by thickening of the skin as well as internal organs), some types of lupus and clogged arteries. During a four-year period from 2013-2017, the researchers analyzed patient data from the Johns Hopkins electronic medical record system (Epic) of 487,104 black women ages 18 and over. The prevalence of those with fibroids was compared in patients with and without CCCA.

The findings translate to a fivefold increased risk of uterine fibroids in women with CCCA, compared to age, sex and race matched controls. Aguh cautions that their study does not suggest any cause and effect relationship, or prove a common cause for both conditions. €œThe cause of the link between the two conditions remains unclear,” she says. However, the association was strong enough, she adds, to recommend that physicians and patients be made aware of it.

Women with this type of scarring alopecia should be screened not only for fibroids, but also for other disorders associated with excess fibrous tissue, Aguh says. An estimated 70 percent of white women and between 80 and 90 percent of African-American women will develop fibroids by age 50, according to the NIH, and while CCCA is likely underdiagnosed, some estimates report a prevalence of rates as high as 17 percent of black women having this condition.
